First Passport Application for U.S. citizens Under the Age of 16

You may only apply for a U.S. passport if you are already in possession of one of the following documents:

  • U.S. birth certificate
  • Consular Report of Birth Abroad
  • Certificate of Naturalization
  • Certificate of Citizenship

If you have one of the above documents but have never had a U.S. passport, please follow the instructions below:

Step 1:

Fill out and print the DS-11 Application for a U.S. Passport form.  If you experience trouble with the Passport Application Wizard, click here for Form DS-11 [PDF – 1,004kb].

  • Check that you have read the Privacy and Computer Fraud and Abuse Acts Notices and Disclaims and click submit.
  • Click Apply Online.
  • Print the DS-11 form upon completion.
  • Please note that some instructions and options on the form are only applicable in the United States.

Step 2:

Click here to schedule an appointment.  Use a valid email address and print your confirmation.

Step 3:

Both parents must provide consent authorizing passport issuance for a minor under age 16. Please click here for the Two Parent Consent Requirement for Passport Issuance to Minors Under Age 16. See the scenarios below, and follow the instruction that best applies to your circumstance:

Both Parents MUST:
  • Appear in person with the minor
  • Sign Form DS-11 in front of a Consular Officer/Agent
One Parent MUST:
  • Appear in person with the minor
  • Sign Form DS-11 in front of a Consular Officer/agent
  • Submit the second parents’ notarized Statement of Consent (Form DS-3053).  The form must be duly notarized by a U.S. commissioned notary or a Consular Officer of the U.S. Embassy or Consulate.
One Parent
(with sole legal custody)
MUST:
  • Appear in person with the minor
  • Sign Form DS-11 in front of a Consular Officer/ Agent
  • Submit primary evidence of sole authority to apply for the child with one of the following:
    • Minor’s certified U.S. or foreign birth certificate listing only the applying parent
    • Consular Report of Birth Abroad (Form FS-240) or Certification of Birth Abroad (Form DS-1350) listing only the applying parent
    • Court order granting sole custody to the applying parent (unless child’s travel is restricted by that order)
    • Adoption decree (if applying parents is sole adopting parent)
    • Court order specifically permitting applying parent’s or guardian’s travel with the child
    • Judicial declaration of incompetence of non-applying parent
    • Death certificate of non-applying parent

NOTE: If none of the above documentation is available, the applying parent must submit Form DS-3053 stating why the non-applying parent/guardian’s consent cannot be obtained.  The form must duly notarized by a U.S.- commissioned notary or a Consular Officer of the U.S. Embassy or Consulate.

A Third Party
(in Loco Parentis applying on behalf of a minor under the age of 16)
MUST:
  • Submit a notarized written statement or affidavit from both parents or guardians authorizing a third-party to apply for a passport. The document must be duly notarized by a U.S.-commissioned notary or a Consular Officer of the U.S. Embassy or Consulate.
  • When the statement of affidavit is from only one parent/guardian, the third-party must present evidence of sole custody of the authorizing parent/guardian.

IMPORTANT NOTE: The written consent from the non-applying parent that accompanies an application for a new passport must be less than 3 months old.  New written consent from the non-applying parent must be obtained and submitted with any future passport application for the minor under age 16.

Step 4:

Please bring the following documents to your appointment:

  • Proof of U.S. citizenship which may be any of the following: U.S. Birth Certificate*, Consular Report of Birth Abroad*, Certificate of Naturalization, or Certificate of Citizenship
  • Proof of Parentage listing the names of the child’s parents (e.g. Philippine Birth certificate from National Statistics Office, U.S. birth certificate or Consular Report of Birth Abroad)*
  • DS-11 form (completed but not signed)
  • DS-3053 form (if applicable) or primary evidence of sole authority to apply for the child’s passport (e.g. court order*, death certificate*, etc.)
  • Printout of appointment confirmation
  • Sequential or “growing up” photos for the applicant that cover the time between the last passport and the present.
  • valid photo I.D. of the child’s parents/guardian (e.g. passport, driver’s license, etc.)*
  • Two passport photos
  • $105 in cash (U.S. dollars or Philippine peso equivalent) or credit card

*Also bring one photocopy along with the original. Otherwise, you will be charged for the photocopying fee of $1.00 per page at the Embassy.
